
Short Stay Takamori no Sato
Eligible users
As a general rule, the service is available to those who have been certified as "needing assistance" or "needing nursing care" as a result of the nursing care certification. In emergency situations, even those who have not yet received nursing care certification can use the service.
To apply for short stay, please contact Takamori no Sato through the Totsukawa Village Community Comprehensive Support Center (for those requiring support) or a care manager who will create a care plan for those wishing to use the service.
VISION
Short stays are a form of "community life support."
We provide a combination of helpers, day care services, and short stay services to help residents persevere in their own homes. As a stronghold to ensure residents can continue living in Totsukawa Village until the very end, we strive to work in cooperation with other parties to provide a service that users and their families can trust.

Point ①
Small grouping
Starting in July 2024, we will divide the existing special nursing home with 36 residents into two groups of 18 residents, and put in place a system to make it easier to have a deep relationship with each resident. By having a deep relationship with each resident, we can get to know them and provide support (individualized care) to help them live a life that is true to themselves.

Independent living support care
We will continue to learn about independent living support care together with Kaoru En in Shintotsukawa Town, and will continue to incorporate and put into practice new knowledge and information every day in order to provide better care.


Point ③
alignment
We will work closely with families and the care managers in charge to report and communicate on the progress and condition of residents during their short stay, and provide appropriate care to reassure family members.
BASE SYSTEM
1. Meals
A registered dietitian creates menus with nutritional balance in mind. We are as flexible as possible to accommodate individual dietary restrictions and food preferences. You can enjoy your meal at a table away from your bed.


2. Exercise
The environment is larger than your home, so you will naturally increase the number of steps you take and the amount of exercise you do. You can exercise without straining yourself by taking walks inside and outside the facility or participating in recreational activities.
When the weather is nice it's very pleasant to sunbathe in the courtyard.
3. Pick-up and drop-off
We provide transportation between your home and the facility. We can also provide transportation for people with disabilities, using vehicles that can be accessed from a wheelchair or with lift-up seats. For people who cannot walk or sit up, staff will transport them to their beds at home.

Please pay the amount (self-payment amount) that is the service usage fee according to the policyholder's level of care required minus the nursing care insurance benefit amount, based on the table above. (The service usage fee varies depending on the policyholder's level of care required and the room type.)
*If the policyholder has not yet been certified as needing long-term care, the full amount of the service usage fee must be paid at once. After certification is received, the amount excluding the self-payment amount will be reimbursed by the long-term care insurance (reimbursement payment). If reimbursement is required, the policyholder will be issued a "Service Provision Certificate" that lists the items necessary for applying for insurance benefits.
*If there is a change in the amount of your nursing care insurance benefits, your copayment amount will be adjusted to reflect the change.
*Breakfast is 390 yen, lunch is 535 yen, and dinner is 520 yen (total 1,445 yen). Fees will be charged according to usage.
*Meal expenses for specially prepared meals or eating out at the individual's request will be charged at actual cost and may exceed the amount stated above.
